ALTERATION

AGREEMENT

ALTERATION AGREEMENTS are documents designed to make sure that the building-wide rules, services and structures that serve all residents are not affected by construction in any one apartment. 

Prior to any work being done in a unit, the owner/hiring party must submit an Alteration Agreement Notice via email to their respective boards and management.  That notice will be reviewed and approved by
THE REFERRED TO ASSOCIATION BOARD and will be filed with E.T. Property Management, Inc.

ALL vendors scheduled to perform work in any one unit or in any part of the building MUST complete an Alteration Agreement, submit the requested scope of work, permits, licenses, and certificate of insurance. 

Required Documentation - Submit forms to MAINTENANCE@ETPROPERTY.MANAGEMENT:

 

  1. The owner/hiring party must submit an Alteration Agreement Notice that has been approved and signed by THE REFERRED TO ASSOCIATION BOARD and has been filed with E.T. Property Management, Inc.
     

  2. The contractor MUST PROVIDE  to E.T. Property Management, Inc. 

    • Scope of work

    • Signed alteration agreement CLICK HERE TO DOWNLOAD

    • Permits

    • Licenses of ALL contractors working on the renovation - General Contractor, Electrician, Plumber, etc.

    • Certificates of Insurance (Worker's Compensation and Liability insurance) for  ALL contractors working on the renovation - General Contractor, Electrician, Plumber, etc.

    • Certificates of Insurance (Worker's Compensation and Liability insurance)  must be provided & filed for:

  • Unit Owner(s) - Full Names and Address

  • The Condo Association

  • E.T. Property Management, Inc  134 8th Street Brooklyn NY 11215

Fees - Project Management:
 

  • Paid by the owner of the unit being renovated, where applicable

  • $100 each site inspection  - Periodic inspections scheduled by the property manager to ensure contractors and vendors are meeting NYS building codes, association rules, and general safety practices. Applied as needed basis to the unit owner. Inspection schedules where applicable: Before, After Demo, Electrical, Plumbing, Framing, & Completion of Project.

Certificate of Insurance Guidelines

COI's are required anytime a vendor is hired to perform work or service within the building or a unit. This includes but is not limited to moving, renovation, painting, repairs, some inspections, major appliance & furniture deliveries. If you have questions about whether or not you will need to submit a COI you can contact us at maintenance@etproperty.management 

The hiring party is listed as the certificate holder. 

All other parties listed below are listed as additional insureds.

Each occurrence limit must state $1,000,000

 

COIs [Certificates of Insurance] should be created & filed for all 3 entities below:

  • Unit Owner(s)   (i.e. Earl E. Bird 123 Main Street Unit 2 Brooklyn, NY 11201)

  • The Condo Association Name (i.e. 123 Main Street  Condominiums 123 Main Street Brooklyn, NY 11201)

  • ET Property Management, Inc 134 8th Street Brooklyn NY 11215
